Here is the NSFAS 2025 Applications consent form

As part of the requirements to apply for NSFAS funding, you need a NSFAS Applications consent form that must be filled in full by your parent/guardian/spouse.

This is because NSFAS reserves the right to validate all information and details that you and your parent/guardian/spouse have provided against independent third-party data sources.

Before filling in the consent form, please take note of the following;

• You must complete the consent form in capital letters
• You must fill out the form completely in detail, in legible handwriting, in black ink
• Do not use correction fluid on the form
• Do not use electronic signatures on the form
• All corrections are to be initialed by all parties
